Do I Need a Referral for Physiotherapy in Ontario?

The short answer: most patients can book private physiotherapy directly in Ontario without a doctor’s referral. At Symphony Physiotherapy, you can contact us or book an initial assessment online without seeing a physician first.

There are still a few situations where a referral or supporting documentation may matter. The distinction usually comes down to where you receive care and how the treatment will be paid for.

Can I book private physiotherapy directly?

Yes. For physiotherapy provided outside a hospital, the College of Physiotherapists of Ontario does not require a physiotherapist to have a referral before assessing or treating a patient. This direct-access model means you can arrange an appointment when pain, stiffness, weakness, dizziness, or an injury begins affecting your daily life.

You do not need to know your exact diagnosis before calling. Your first appointment is designed to review your health history, symptoms, movement, function, and goals. Your physiotherapist will determine whether physiotherapy is appropriate and will recommend another health professional when further investigation is needed.

Could my insurance plan still require a referral?

Possibly. A clinic may allow direct booking while an extended health benefits plan has separate reimbursement rules. Some plans cover physiotherapy without a physician’s note; others require a referral before they will reimburse a claim.

Before your appointment, check your plan booklet or insurer portal for:

  • Your annual physiotherapy coverage limit
  • Whether a doctor’s referral is required for reimbursement
  • Any per-visit maximum
  • Whether the plan permits direct billing
  • Whether a new referral is required each benefit year

A referral requirement from your insurer is about benefit eligibility—it does not usually determine whether a private physiotherapist can assess you.

What about government-funded physiotherapy?

Government-funded care follows its own eligibility and program rules. Ontario’s Community Physiotherapy Clinic Program allows eligible patients to contact a participating clinic directly; a 2025 Ministry of Health bulletin confirms that primary-care referrals have not been required for that program since April 1, 2024.

Eligibility is separate from referral status. Ontario currently lists categories including people aged 65 or older, people aged 19 or younger, eligible Ontario Works or Ontario Disability Support Program recipients, and people needing physiotherapy after a qualifying hospital stay or outpatient surgery. When should I see a doctor first?

You can often begin with a physiotherapy assessment, but some symptoms need urgent medical attention. Call 911 or seek emergency care for severe or rapidly worsening symptoms, chest pain, difficulty breathing, signs of stroke, a suspected major fracture, loss of consciousness, or loss of bowel or bladder control.

You may also want to speak with your primary-care provider if you need medication review, diagnostic imaging, a specialist referral, or documentation required by an insurer or workplace.

What should I bring to my first appointment?

  • Comfortable clothing that allows you to move
  • Your insurance information, if you plan to use benefits
  • Relevant reports or imaging results you already have
  • A medication list and key medical history
  • Your WSIB or auto-insurance claim details, when applicable
